Rural Health Group, Inc. is an FQHC with nine locations serving over 23,000 patients in six counties in NC (North Hampton, Halifax, Warren, Nash, Vance, and Warren) and providing comprehensive integrated practice that includes behavioral health and dental services.

 

Freedom House Recovery Center is a non-profit organization located in Chapel Hill, NC that provides substance abuse and mental illness services to Orange County residents.

 

Easter Seals UCP North Carolina & Virginia, Inc. is a CABHA that provides services in NC and Virginia to people managing disability and mental health problems.

 

Duke Family Care is a program of Duke University Medical Center providing services to women with substance abuse problems who are improving their families, regardless of ability to pay.

 

SouthLight is a private, non-profit, United Way-funded CABHA located in Raleigh, NC. It serves Wake County and provides comprehensive substance abuse programs addressing the needs of youth, adults, and families.

 

Carolina Family Health Centers, Inc. provides primary health and preventive services in Edgecombe, Nash, and Wilson Counties of eastern NC.

 

Guilford Adult Health is a six-site Patient Centered Medical Home practice serving the people in Guilford County.

 

Lincoln Community Health Center, a BAART program located in Durham, NC, is a non-profit organization that provides primary care, preventive health services, behavioral health and focused case management to indigent populations in Durham County.

 

Robeson Health Care Corporation is an FQHC non-profit organization that services the rural areas of Robeson County. Comprehensive health services are provided to individuals, regardless of ability to pay, at five primary care medical clinics.

 

Kinston Community Healthcare Center is an FQHC that provides comprehensive dental, family medicine, and OB/GYN services in Lenoir County.

 

CenterPoint Human Services is a state-mandated Local Management Entity (LME) that oversees publicly-funded mental health, developmental disabilities and substance abuse services in Forsyth, Stokes, Davie, and Rockingham Counties. The mission is to develop, manage, and coordinate collaborative partnerships and contracting with Daymark Recovery Services, Inc. for co-location projects.

 

Coastal Horizons Center, Inc. is a private, non-profit CABHA serving eastern NC and Durham County. They provide services for children, adults, and families that span physical, emotional, and social development.

 

Western North Carolina Community Health Services, Inc. is a private, non-profit FQHC that is located in Asheville, NC and serves Buncombe County. It provides medical, dental, and behavioral health services.

 

Wilmington Health Access for Teens (WHAT) is a non-profit pediatrician’s office that provides medical, mental health, dietary, and health education to adolescents serving New Hanover County.

 

Bakersville Community Medical Clinic, Inc., located in Bakersville, NC, primarily serves Mitchell and Yancy Counties. The clinic provides the community with family practice, nurse-midwifery, chiropractic, and acupuncture services. The five-physician team provides a spectrum of primary care at the clinic and inpatient care at Spruce Pine Hospital. The clinic serves as a teaching site for medical students sponsored by the Office of Rural Health.

 

Johnston County Mental Health Center is located in Smithfield, NC and provides services for mental illness, developmental disabilities, and substance abuse to the people of Johnston County.
